Health authorities around the world can’t seem to agree on how to deal with e-cigarettes since there is not enough research data available on these battery-driven devices. A few countries have banned them outright, such as

• Brazil
• Norway
• Singapore

while others are opting for varying degrees of regulation, including limits on advertising and curbs on their use by minors and in public places.

Sales of e-cigarettes have boomed in recent years, prompted by tighter restrictions on smoking in public, higher tobacco prices, health concerns over secondhand smoke, and the social stigma attached to the habit. But the long-term effects of ecigs are as yet unproven and some doctors argue their popularity could undermine years of anti-smoking efforts, with particular concerns about promotion to children and non-smokers.

Research suggests around 1.3 million smokers and ex-smokers in the UK use electronic cigarettes, which are designed to replicate smoking behaviour without the use of tobacco. They turn nicotine and other chemicals into a vapour that is inhaled.

Jeremy Mean of The Medicines and Healthcare Products Regulatory Agency (MHRA) said the government had concluded that e-cigarettes currently on the market do not meet appropriate standards of safety, quality and efficacy. Contaminants were found in some products and nicotine levels often vary.

“We can’t recommend these products because their safety and quality is not assured, and so we will recommend that people don’t use them,” Mean said at a news conference.

The MHRA had decided not to ban the products entirely but to work towards a position where they are licensed.

“Smoking is the riskiest thing you can do – we want to enable people to cut down and quit – we don’t think a ban is a proportionate action.”

Action on Smoking and Health (ASH), a health campaign organization, said the new regulation will ensure that “adult smokers can continue to be able to buy e-cigarettes as easily as tobacco, but promotion to children or non-smokers will be prohibited.”

Professor Dame Sally Davies, Chief Medical Officer, said with more people using e-cigarettes it was only right that the products were properly regulated to be safe and work effectively.

“While it’s best to quit completely, I realise that not every smoker can and it is much better to get nicotine from safer sources such as nicotine replacement therapy,” she said.

Manufacturers of e-cigarettes claim that vaping products have the potential to save lives and should not be restricted.

Adrian Everrett, chief executive officer of E-Lites, told the BBC: “So far not one person globally has been killed by an electronic cigarette and yet every 5 minutes in this country alone someone dies from tobacco use.

“To remove or restrict the use or availability of the electronic cigarette from this market would be a significant health loss.”

Under the new British system, manufacturers will have to prove the quality of their products and demonstrate that they deliver the correct amount of nicotine. But they won’t be required to conduct clinical trials.

Electronic cigarette companies will have enough time to raise their game and apply for a license by 2016. In some countries, such as New Zealand, e-cigarettes that are regulated as medicines can only be purchased in pharmacies. Named for the popping sound that is made by the burning cloves, kreteks are consumed by 93% of all smokers in Indonesia because they are usually cheaper than “white” cigarettes. It said the FDA should be “encouraging and supporting studies” into e-cigarette safety and their usefulness as a “smoking replacement product.” It also calls the FDA’s original study “misleading,” noting that the FDA stated that e-cigarettes contain carcinogens “without bothering to mention that FDA-approved nicotine replacement therapy products contain equivalent quantities.” the CASAA claims that a gram of the liquid found inside e-cigarettes contains the equivalent carcinogens of a nicotine patch, and less than one percent of those found in a full-flavor Marlboro cigarette.

The FDA has continued to face problems regulating e-cigarettes because of the way the devices are classified. If they are not marketed as something that can help you quit smoking, then they are not subject to the same laws covering nicotine patches or gum. “What is the appropriate safety standard for nicotine delivery?” asks Bates. “Most is delivered in a matrix of toxic smoke and poisonous gases.” If e-cigarettes are less harmful than tobacco, why not embrace the technology and attempt to improve it?
